The co-pay is a contractual agreement between you and your insurance carrier that is collected before every visit when you check in.  There are some instances when the insurance carrier may process your claim without a co-pay.  In such cases, your copay will either be refunded to you or credited to your account to be used for future co-pays or outstanding balances.
