| How can I obtain a copy of my child's medical record? | |

We are happy to provide you with a copy of your child’s medical records. We do ask that you pick them up in one of our offices due to problems in the past with records getting lost in the mail or sent to the wrong location. If you have moved from this area and are transferring your records out of town, we will be glad to mail the records to your home address. A parent or legal guardian of the child must fill out the medical record release form for all children under 18 years old. If you are the legal guardian for a child, please be prepared to provide us with the necessary documentation to verify your guardianship. Please allow us up to 2 weeks to prepare your child’s medical records. Once the medical records are prepared, we will hold them for 60 days. There is no charge for producing a copy of your child’s medical records the first time; however, subsequent copies of the medical records are subject to a fee to cover materials and handling.
