We will gladly fill out physical or day care forms if your child has had a current well-child checkup at our office, and the child's immunizations are up to date. Please note that sick visits do not count as well-child visits. Click here to see when well-child checkups should be scheduled.  A physician will sign the appropriate physical forms for children over 2 years of age who have had a well-child checkup within the last year.  If a child has not had a well-child checkup within the last year, you will be asked to schedule a checkup before the physical form can be completed.  Please allow at least 24 hours to complete the form.
