Yes, we welcome new patients from birth to 18 years of age. Be sure to check the list of insurance companies with which we participate.

 New patients may choose to print the following forms to be filled out prior to your appointment.  The patient forms are located on the Patient Forms tab located on the main page of the website.  Please bring the completed froms with you to your first appointment.  Call us if you have any questions about completing these forms.  If you have commercial insurance you will need to complete the first two forms: the patient information sheet and the privacy policy form.  If you have insurance through the State of Missouri (i.e. Medicaid) you will need to complete the VFC form as well.
